This has most often been my experience. When I first started getting interested in Wagner (which wasn't that long ago), I went to the library one day and was ecstatic to see that they had Furtwaengler's EMI recording of Tristan. I came home and got prepared for a dive into decadence, only to discover that the discs were in such bad shape that they were barely playable. One of the discs was so badly scratched that it wouldn't play at all, and the other three all skipped. I can only assume that Nietzsche had borrowed them at one point and attempted to break them in half.

Anyway, shortly after that, my attempts at music piracy having been foiled, I had to actually buy the Furtwaengler set myself. Oh, the horror!

I've had better experiences on the whole. Some ten years ago I borrowed both the Boehm ring and the Boulez ring to make copies on cassette. I didn't have any problems at all. The same with the Levine/Bayreuth Parsifal.

The way I've generally seen the term used is in reference to recordings of music composed specifically to be hired out to use in background scores for various kinds of film or radio productions when the producers of same, for whatever reason, do not want to pay to have music specifically composed and recorded for them. These uses could be for commercials, feature films, documentaries or short subjects. Maybe that's the context in which you've heard the term.

Do you suppose the RIAA sends agents to libraries to gouge the discs?

3) The library coding stickers have been attached to the CD case so as to make it impossible to tell who the artists are or what the music is.

I make regular rounds of the libraries in my area. Most of the CDs are in flawless condition (one branch's CDs look like they've been
